A four part pre-printed form on red paper, pages 1 and 2 are the Bond for the rural carrier substitute applicant, whose bond is guaranteed by two local residents. The bond is signed by the applicant, the two bondsmen, the postmaster and a notary public with his embossed seal. Page 3 is the Oath of office, and the 4th page is a Document identification box.
